A guide to viewing, creating, and editing forms in the Group Platform.

Shared Forms on the Group Level

Shared Forms can be:

  • Created

  • Edited - full editing capabilities

  • Viewed/Printed

  • Duplicated

  • Archived - if archived on the Group Level, the form will not be visible on the Clinic Level

Shared Forms on the Clinic Level

Shared Forms can be:

  • Sent to clients

  • Edited - can only add/edit Appointment Types for automatically sending forms when an appointment is created

  • Viewed/Printed

  • Archived - Clinics can archive forms that are not applicable to them. If archived on a Clinic Level, the form will still be visible to the other Clinics and in the Group Platform.
