If your client needs to prove that a patient has been spayed or neutered, here's a sterilization form you can use.

Shepherd does not automatically generate a sterilization certificate at this time. However, you can use the form located here to enter the patient's information and generate a form that can be printed.

Create a Certificate

This button will take you to a screen where you'll need to:

  1. Select the 'Start filling' button

  2. Fill out the applicable fields for the animal, client, and sterilization information

  3. Select 'Preview PDF'

4. Download the form as a PDF, and then

a. Upload it into the SOAP

b. Print

c. Attach and send the form as an email
