Lists identified Problems, Diagnoses, and Chronic Conditions for patients.

The Problem and Diagnosis Report provides a structured overview of patient diagnoses and associated clinical conditions recorded within a selected time period. It is designed to support medical staff, technicians, and administrators in tracking clinical trends, evaluating chronic cases, and ensuring consistent documentation across patients.

⚠️ Note: This report is for operational review and clinical tracking.


🧭 How to Use This Report

1. Filtering the Report

Use the filters at the top of the screen to tailor the data:

  • Time Period: Select the desired reporting window (e.g., "This Month").

  • No other filters are required, but data can be exported for further processing.

2. Report Data Structure

The report is divided into two major data views (seen across two panels):

Column

Description

Problem

Presenting complaint or issue (e.g., Limping, Vomiting).

Diagnosis

Clinical diagnosis confirmed during the SOAP note.

Chronic

Lists chronic or ongoing conditions linked to the patient.

Date of SOAP

The date of the examination and record entry.

SOAP ID

Unique identifier for the SOAP note (internal reference).

Supervising Doctor

Veterinarian responsible for oversight and SOAP entry.

🩺 Patient & Demographic Details

Column

Description

Client Full Name

Pet owner's full name.

Client ID

Unique identifier for each client.

Patient Name

The patient involved in the record.

Patient ID

Internal unique ID for the patient.

Species

Animal type (e.g., Canine, Feline).

Breed

Specific breed.

Patient Age

Age of the patient (Years & Months).

Sex

Biological sex and status (e.g., FS = Female Spayed).

You can export the report as a CSV or PDF file for:

  • Manual backups

  • Offline analysis

  • Import into practice management systems

To export:

  1. Click the "Download" button.

  2. Or, select "Problem and Diagnosis Report CSV" at the bottom of the interface.

  • Review regularly: Weekly or monthly reviews help identify patterns (e.g., repeated diagnosis of chronic issues).

  • Use for staff training: An excellent reference for SOAP note accuracy and consistency in diagnostic labeling.
