Delete a Chronic Condition from a SOAP

Sometimes chronic conditions get resolved—other times they just need to be deleted. Here's how you delete a chronic condition from a SOAP.

Deleting a Chronic Condition from Inside a Medical Record

If there's a chronic condition that's been added to a record by mistake (or that you just need to delete):

  1. Open the SOAP

  2. Navigate to the assessment section (the 'A' tab)

  3. Click the 'X' in the upper right hand corner of the chronic condition

  4. Add a note for why you're removing it (i.e. 'Added by mistake')

  5. Click the 'Delete instead of Resolve' button

  6. Confirm (in a popup)
