Add/edit discharge instructions or activating/deactivating diagnoses to ensure consistent patient care.

The Diagnoses section of Shepherd’s Admin contains all of the diagnoses that can populate within the ‘Assessment’ section of a patient’s SOAP. Shepherd uses the American Animal Hospital Association (AAHA), American Association of Equine Practitioners (AAEP), and Small Animal Specialty (SAS) subsets from SNOMED codes.

To access the Diagnoses section, navigate to Admin -> Diagnoses

Viewing a Diagnosis

To see specifics about a diagnosis, select the ‘view’ icon (looks like an eye) on the right side of the screen under the Actions menu.

Viewing a diagnosis allows you to see its:

  • Preferred Description

  • Acceptable Descriptions

  • SNOWMED CT Concept ID

  • Subset (AAHA, AAEP, SAS)

  • Status

  • Discharge Instructions

Editing a Diagnosis

To edit a diagnosis, select the ‘edit’ icon (pencil icon) on the right side of the screen under the Actions menu. When editing a diagnosis, you’ll be able to activate or reactive it and add discharge instructions.

Adding Discharge Instructions to a Diagnosis

Adding auto-populating discharge instructions to a diagnosis is a huge time saver. After the discharge instructions have been saved, they’ll populate whenever the diagnosis is added to the patient’s SOAP.

To add custom discharge instructions to a diagnosis you will select 'edit' to open the diagnosis. The discharge instruction box is a fully formattable text field, so you can edit everything from font size to color and even add pictures.

There are pre-populated fields available to insert into the discharge instructions to help make them more personable without needing to manually edit them in a SOAP. For example, when using {PATIENT NAME} it will automatically populate that patient's name into the field on the discharge instructions.

You can also add an existing PDF file to populate with discharge instructions.
